Interest Form and Contract


   A non-refundable registration fee of $60 per child is required for enrollment. Payments can be accepted in the form of cashier’s check, personal checks, and money orders if paid in person at one of the IELC’s or thru the ProCare App which will require to be downloaded when registered.   After school tuition is due by Thursday of that week, if not received by 9 a.m. Friday, a late fee payment of $25 will be assessed.  Regretfully, we cannot allow children to return to the program until the account has been paid in full.  A $25 return service fee will be applied to all returned checks. 


All fees are non-refundable.

Parents will be charged a $1 late pick up fee for every minute your child stays past 6:00 pm.

This fee doubles to $2.00 per minutes beginning with second occurrence.

Child Admission and release requirements: 


Parents/Guardians are required to sign their children in and out of the program at pick up time. The TUSD Community School staff will sign the children into the program upon dismissal of school. Only persons listed on the children’s emergency form will be able to pick their children up after-school with a proper picture I.D.  Telephone authorizations will not be accepted as last minute pick up persons.

 Age-appropriate Discipline Guidelines:

In the Lineweaver Programs, discipline involves teaching children how to get along with others. 

         Staff uses positive reinforcement, redirection, logical and natural consequences, and other appropriate methods to help children learn how to manage their own behavior. 

         Your child will participate in activities that provide him/her with opportunities to learn and practice self-control, cooperation, and making healthy and positive choices.

         It is against the law for any staff to use discipline that is physically or emotionally abusive. 

         Staff will share any concerns about your child’s behavior with you.  If needed, a family conference will be scheduled to discuss strategies that will best benefit your child.

Transportation:


          Parents are responsible for transporting children to and from home.  Staff are not permitted to transport children in their own vehicles.

         Transportation is available as a related, specialized service for students who qualify for an Individualized Educational Program (IEP) and will be documented on.

Field Trip Requirements:


          Field trips will be announced in advance and require parent/guardian permission on designated permission forms.

         Description of Activities and Program:

         TUSD provides a variety of activities.  These activities may include outdoor play, reading, art projects, dramatic play, computers, sports, games, nutrition education, science, cooking, puppetry, singing, music and movement.

         Liability Insurance:

         Lineweaver Community School carries liability insurance through the Tucson Unified School District

         Accident & Emergency Procedures:

         All staff are CPR/1st Aid trained and trained on emergency evacuation procedures.

         Evacuation drills are conducted monthly and recorded on a Fire Drill Report

         TUSD Community Schools follows the T.U.S.D.’s Emergency Guide procedures.

         Parents will be notified using the information provided on the child’s Emergency, Information and Immunization Record Card

         TUSD Medication Policy:

        Over-the-counter medications such as aspirin, Tylenol or cough syrup cannot be given.

        Prescription medication must be given only as prescribed by your child’s Doctor, and will be administered by Site Lead or designee staff member in her absence.

         You must fill out a medication consent form which accompany all medications at all times.

         All medications will be kept locked in a locked closet in a locked box. Must be original container with a prescription number and the Doctor’s name.

         Epi-pens are kept with the teacher or staff, who has received training.

         If antibiotics have been prescribed for your child, please do not send them to school until they have been on the medication for a full 24 hours.

         Do not send any medications, such as cough drops, to school with your child.  The classroom staff cannot give these to your child.

        Pesticide Notification:

         Parents will be notified by posting of application of a pesticide 48 hours prior to application.

Parental Access Parents have access to the areas on facility premises during operating hours.
